I'm confident that over the past decades people have proposed alternative
approaches to the use of headers and preprocessor directives.  I tend to
include namespaces in a grouping with the other two elements of C++, but
that isn't necessary for this question.

Where might I find a discussion of proposed alternatives to the use of
header files and #includes?
