archie finds it for me at:

   ftp.huji.ac.il:/pub/doc/misc/c++-rules.ps.gz

It is probably identical to the copy you have, as the authors are working
on a new version for publication as a book, rather than being freely
available on the net.

By the way, anyone have any info on "Real-Time" programming on Unix (AIX) or the AIX so-called
