Topic: Material or examples combining design, analysis, and coding in C++

There are books that teach syntax, and then there are books that teach
design.  Does anyone know of material suitable for intermediate to
advanced levels that combine both, i.e.,

     design and analysis phase, using Rumbaugh (or other
        notation)
     moving to *completely* coded examples

When structured programming is taught, students do structure charts,
flowcharts and pseudocode before actually coding.  This helps them to
think abstractly and independently of the language.  It seems to me that
object oriented programming in C++ should be done in a similar fashion.
